How to Make a Referral

We invite referrals and pre-referral consultations from parents, guardians, health professionals and legal representatives. Our Intake Manager will be pleased to assist you. Members of the legal community who make referrals to us are already familiar with the initial information we require to create our case file. If you are calling us directly, it is helpful to have the following information:

• Child’s first, middle and last name, and date of birth
• Reason for referral and the nature of physical, cognitive or emotional needs
• If services are insured, the name of the insurance company(ies), contact person (if applicable) and the insured’s policy and member identification number
• Parent or substitute decision maker’s name, contact information and address
